How were the LM Ascent Stages De-orbited?

When re-docked with the CM, the LM Ascent Stage was then de-orbited to crash into the moon, or sent into a solar orbit. How was it de-orbited? When the LM (ascent stage) and CM undocked, did the LM ascent stage fire? I can't find records of this anywhere. I know the orbit didn't just decay, it would've taken a year (like Apollo 16's LM). I remember reading somewhere though that Apollo 16's LM ascent stage didn't de-orbit due to a technical malfunction, which was caused by not flipping a switch to signal the de-orbit burn, but I can't remember where this was.
